clearance packets must be turned in before an athlete can compete for Poly (make sure you follow the directions). We
ask that student-athletes get the appropriate signatures (bookroom,nurse, banker) during their registration day at Poly H.S.
Once you have the necessary signatures, turn in your completed clearance form (make sure you have a copy of your insurance
card) to room 105.
